Registration 

MARQUARDT SCHOOL DISTRICT 15

Marquardt School District 15 requires that parents/guardians submit a Proof of Residency packet every school year. After proving residency, parents/guardians must also complete a Registration Packet for their child in order for him/her to start the school year on time. If you are a parent/guardian, please complete Residency & Registration by following the four steps below so that your child can start school on-time next year. 

Is your child new to Marquardt School District 15? In addition to Proof of Residency, new students will need to provide a certified copy of a birth certificate and if transferring from another school, an Illinois State Transfer form.


STEP 1: Identify Your Living Situation

STEP 2: Get a Residency Packet

OPTION B: 

Pick up a residency packet at your school office or the District 15 office.

OPTION C:

Request for a residency packet to be sent home with your child by contacting your school's main office.

 

STEP 3: Complete Proof of Residency Packet

 

Returning Students

Schools will begin accepting completed Proof of Residency packets for the 2017-18 school year on Monday, March 20th, 2017 during parent-teacher conferences.

new students

Parents of new students must bring the completed residency packet and birth certificate to their home school. Please see Boundary Map if you need help in determining your home school.

Incoming kindergartners

Parents of incoming kindergartners will need to attend Kindergarten Registration on April 10th, 2017, from 4:30-7:30 pm.

  • Registration will take place at all four elementary schools. Please go to the location closest to your residence.
  • You must bring the completed residency packet and birth certificate to Kindergarten Registration.
  • If you are unable to attend, please go to your home school between 10:00 am and 2:00 pm before the end of the current school year to register.
 

STEP 4: Complete & return Registration Packet

Once your Proof of Residency forms are accepted and approved, you will receive via mail a 2017-18 Registration Packet. Please complete forms and return them to your child's school as soon as possible. Families whose forms are received by April 30th, 2017 qualify for a school fee discount.

All registration paperwork will be completed on site.
