School Registration

New Student Registration

Parents of new students are encouraged to enroll their children as soon as possible after moving into District 15. The registration packet for new students includes the following forms that the parent/legal guardian must complete at the time of registration:

• Application for Free & Reduced Lunch
• AT & T Broadband Cable Release Form
• Authorization for Release and/or Exchange of Information
• Directory Information
• Fee Form
• Home Language Survey
• New Student Information
• Student Request for Loan of Textbook

Additionally, the parent/legal guardian must provide the following information to complete the enrollment process:

Proof of residency
Parents, guardians or persons asserting their legal custody of incoming students must provide documented proof of their residence within the boundaries of Marquardt District 15. Administrators may also require proof of residency at other times during the school year in order to clarify any questions regarding this issue. Use the following as proof of residency:

1. Photo identification( driver’s license, Illinois state identification, green card, passport/consulate ID) One required-must be current.
2. Title evidence, Mortgage papers, Lease agreement. One required-must be current.
3. Utility Bill, Tax Bill, Loan Payment book, Home or auto insurance policy, bank/checking account statement. One required- must be current
4. Completed Registration and *Proof of Residency Questionnaire and notarized Affidavit of Residence.

*Only required if parent/guardian is not homeowner/leaseholder. The homeowner/leaseholder of your current residence is required to prove residency.

Student Transfer Form
This form is provided to the parent at the time of withdrawal for students transferring from one Illinois public school district to another.

Proof of immunizations

Continuing Student Registration
In the spring of each school year, pre-printed registration forms are mailed home to the parents/legal guardians of District 15 students. At that time, parents/legal guardians are asked to update the pre-printed information and return it to the student's school by a specific date as a confirmation of the intent to return to District 15 the following fall. An invoice for the consumable fee is also sent home with the registration form.
